A 61 years old woman affected by hypertension, secondary hyperparathyroidism and previous breast cancer in remission was on hemodialysis treatment for 11 years using a left radio-cephalic AVF. In September 2011, a left cephalic vein thrombosis occurred; echo-color Doppler scan showed that thrombosis involved a venous tract of about 20 cm of length including two aneurysmal dilatations distant respectively 7 and 18 cm from arteriovenous anastomosis (AV). Aneurysms area was about 2x2 cm and they were almost completely thrombosed (fig 1a).Different treatments are indicated in case of AVF thrombosis: systemic or regional pharmacologic thrombolysis, mechanical thrombolysis or surgical trombectomy. Thrombosis extension prevented us from performing the last two options. Endovascular treatment with ultrasonic (US) energy combined with thrombolitic infusion deep into the clots is a therapeutic option to treat artery and peripherical vein thrombosis and pulmonary thromboembolism. No description about this treatment option performed on AVF was reported in literature.
